since Josh is very verbal, have you asked him why we doesn't want to poop again? Does it hurt? Does he hate the feel of it? Is he tired of people focusing on it? Does he think it is funny? Perhaps a good old dialogue with him might shed some clarity on how to help him feel comfortable doing his business again.
Once you've ruled out physical discomfort, and if he can understand ifs and thens, you might tell him it's ok if he wants to hold the poop in (and get totally ok with that) but until then he can only eat things that are good for his body and make it easier to poop once he's ready to since holding it in could cause it to harden and make it really uncomfortable for him. Then, cut out all constipating things like rice, crackers, bananas, treats, etc. Now, this is assuming he does eat other things since you don't want him to starve. Also, add lots of magnesium to his diet to keep the stools loose and keep A&D ointment on his bottom to prevent rashes from leaks.
